Multiple Teams · WINTER SPORTS TRYOUTS – EVERYTHING YOU NEED TO KNOW


Believe it or not, it’s time to start thinking about Staley Falcon 2018-19 winter activities!  The first practices are set for Mon., Oct. 29 for wrestling and basketball and Mon., Nov. 5 for girls’ swimming.  Unless you have already turned it in to the school or to your coach, make sure that you bring your physical form COMPLETELY filled out (including parent and doctor signature areas) when arriving to practice or else you will not be allowed to participate!

Here are some FAQ about physicals that we frequently get in the Staley Activities Office:

  1.  WHERE DO I GET A PHYSICAL FORM?  Click here: https://www.mshsaa.org/resources/pdf/PreParticipationPhysicalEvaluation_1617.pdf or get a hard copy in the administrative center at Staley.
  2. WHY CAN’T I UPLOAD MY PHYSICAL TO PRIVIT?  Staley is not using Privit any longer for physicals.
  3. WHERE SHOULD I BRING MY PHYSICAL?  If you did not turn in your physical at last May’s Physical Night at Staley, please wait until the first practice and turn in your completed physical to your coach.  PLEASE MAKE SURE THE STUDENT-ATHLETE AND PARENTS/GUARDIANS HAVE SIGNED ALL PARTS OF THE PHYSICAL!!!  We can’t stress this enough because if it is turned in without the correct signatures, the student-athlete will not be allowed to participate until the signatures are obtained.
  4. CAN I DROP OFF MY PHYSICAL AT THE OFFICE BEFORE ?  No.  Please bring your completed physicals to the first practice to give to your coach at the first day of practice.

Also, be aware that there is a $50 activity participation fee that will be collected by coach (or was earlier collected at Green Carpet Days/school enrollment).  The one-time fee will cover all activities for the entire 2018-19 school year, so if you’ve already participated in a fall activity, you do not have to pay again.

You will also need to turn in the NKC Schools Handbook form and the Impact Concussion parental consent form:
